NCQA recognition for medical homes: What does it mean?

NCQA sets specific standards for primary care practices to use in organizing care around patients, working in teams and coordinating and tracking care over time. The criteria provide an excellent guide to reorganizing care delivery to create a PCMH. This type of healthcare setting facilitates partnerships between individual patients and their personal physicians — and when appropriate, the patient’s family. The PCMH practice uses registries, information technology, online patient portals and other means to ensure that patients get indicated care when and where they need it.

NCQA standards are aligned with the core components of primary care:

  • Access and communication;
  • Patient tracking and registry functions;
  • Care management;
  • Patient self-management support;
  • Electronic prescribing;
  • Test tracking;
  • Referral tracking;
  • Performance reporting and improvement; and
  • Advanced electronic communications.

The standards include must-pass elements considered essential to the PCMH. These elements are required for practices at all recognition levels. Practices get points for each element of the standards they have accomplished. NCQA awards levels of recognition from the scores that practices achieve, with Level 3 being the highest.

Practices seeking PCMH recognition complete an online data collection tool, provide documentation that validates responses and pay a fee based on the number of physicians in the group.

We used the National Committee for Quality Assurance (NCQA) as the recognizing body for the program (non-profit & independent). We are proud to say that NCQA has recognized all 3 participating primary-care practices as Level 2 PCMH (Patient Centered medical Homes) using PCMH 2011 criteria.

Attaining PCMH recognition from NCQA is a source of pride for medical practices. But the real value is in what the designation means to patients. Those receiving care from a recognized PCMH get comprehensive, well-coordinated care; ease in making appointments; and short waiting times to see a physician. They develop personal relationships with their healthcare team. Their medical information is collected and stored electronically for instant access by caregivers. Their chronic conditions are closely monitored; follow-up is frequent and self-management is taught and supported. They enjoy convenient online access to their test results, prescription refills and appointment requests. Referrals to specialists are smooth and information transfer immediate.
Recognition, however, is only the beginning of a journey for continuous improvement and cultural transformation. NCQA’s rigorous standards challenge a practice to examine nearly every aspect of its operations. The evolution to a PCMH is a serious undertaking — one that rewards patients with more coordinated, focused and safer care, and rewards providers with greater satisfaction in practicing medicine.”
